This 2026 snapshot ranks countries by the percentage of all people living with HIV who were receiving antiretroviral treatment in 2025. The table uses the UNAIDS 2026 estimates workbook and includes every country row with a published 2025 coverage value: 127 countries in total. Older observations are not carried forward simply to enlarge the ranking.
The table is a compiled research dataset based on 3 official sources, with row-level source and method notes shown in the ranking table. What ART Coverage Measures
ART coverage measures the share of the estimated population living with HIV that is receiving antiretroviral therapy. WHO defines the numerator as adults and children living with HIV who are receiving ART and the denominator as the estimated number of adults and children living with HIV.
This is not the second 95 of the 95–95–95 cascade. The second 95 uses people who know their HIV-positive status as its denominator. The indicator ranked here uses all people living with HIV, including people who may not yet know their status.
A high percentage indicates broad treatment reach relative to the estimated population living with HIV. It does not directly measure viral suppression, treatment adherence, retention in care, drug resistance, HIV incidence, HIV prevalence or the absolute size of the national epidemic.

Botswana leads with a published estimate above 98%. São Tomé and Príncipe and Saudi Arabia are next at 97%, followed by Lesotho at 96%. Seven of the ten highest-ranked countries are in Eastern and southern Africa.

Metric and unit
Metric: percentage of people living with HIV who are on antiretroviral treatment, all ages. Unit: percent. Higher coverage receives a higher rank.
Target year
Every ranked row uses 2025 from the UNAIDS 2026 estimates workbook. Earlier observations are not substituted for countries with a missing 2025 entry.
Inclusion rule
A country is included when the all-ages ART coverage field contains a published 2025 estimate. Blank and unavailable entries are excluded rather than treated as zero.
Status rule
All 127 rows are official estimates published by UNAIDS. No official forecast or modeled projection is included.
Source hierarchy: the UNAIDS 2026 estimates workbook controls the country values; the UNAIDS 2026 global fact sheet supplies the global treatment benchmark; WHO Global Health Observatory metadata is used for the indicator definition and denominator. Supporting sources are not averaged into the country values.
Ranking method: published estimates are sorted from highest to lowest. Countries sharing the same whole-percentage estimate are ordered alphabetically, ignoring diacritics for tie-breaking only. This tie-break does not imply that one tied country has better underlying coverage than another.
UNAIDS reports Botswana's 2025 ART coverage as >98% without publishing a more precise percentage. The ranking therefore shows the value exactly as reported. Every other included 2025 estimate is 97% or lower.
The median of the 127 included country estimates is 73%. The global benchmark is 78%, but a simple unweighted country median should not be compared with the global figure as if both were calculated the same way: WHO and UNAIDS global estimates are population-weighted rather than an average of country percentages.
Limits: the denominator is estimated, published values are rounded, and national estimates can be revised when surveillance, programme or modeling inputs change. ART coverage does not measure viral suppression, adherence, treatment quality, equity between population groups, incidence, prevalence or AIDS-related mortality.

Notable Pattern
Twelve of the 127 included countries have published ART coverage of 90% or higher. The values then spread widely through the middle of the ranking, with a country median of 73%.
Regional Concentration
Seven of the Top 10 countries are in Eastern and southern Africa: Botswana, Lesotho, Eswatini, Namibia, Zambia, Zimbabwe and Rwanda.
Outlier
Botswana is the only included country published with a greater-than threshold rather than a single whole-percentage estimate. The source value is therefore shown as >98% instead of being converted to an invented exact figure.
What the Ranking Means
This ranking is useful for comparing treatment reach, but it is not an overall ranking of national HIV programmes or health systems. Countries can reach similar ART coverage while differing substantially in diagnosis, retention, viral suppression, prevention, financing and access for key populations.
Small gaps should be interpreted cautiously. Country estimates are rounded and depend partly on estimates of the number of people living with HIV. A difference of one percentage point does not establish a meaningful performance gap when uncertainty and later revisions are taken into account.
The ranking also should not be read as the second 95 of the 95–95–95 targets. This table uses all people living with HIV as the denominator. Treatment among people who know their status and viral suppression among people on treatment are separate indicators.

Ireland had a published 2024 value in the previous series, but the 2025 field is unavailable in the current workbook. This ranking does not carry a 2024 observation into a 2025 table.
Why are countries with missing 2025 values excluded?
Using older observations would mix reference years. To preserve same-year comparability, unavailable 2025 values are omitted rather than replaced with earlier estimates.
Is ART coverage the same as the second 95?
No. ART coverage here uses all people living with HIV as the denominator. The second 95 uses people living with HIV who know their status.
Does high ART coverage mean the same percentage is virally suppressed?
No. Treatment coverage and viral suppression are separate measures. Viral suppression requires its own indicator and denominator.
Why is Botswana shown as >98% instead of 99%?
Because >98% is the value published by UNAIDS. Converting the threshold to 99% would add precision that the source does not provide.
